My app records your mood over the course of a day. You will enter how you are feeling at regular intervals along with some other basic information like how many hours of sleep you received in the last 24 hours. Later, you can look at charts of how your mood has changed over time.
Screen 1: Data Entry screen - you will enter a number between say -15 to +15. Each number will have an adjective with it describing your feeling. A +10 might be "ecstatic". Not sure if you are entering a number on a keypad or a drop down type widget. In addition, you will also be able to enter notes into other fields like "How healthy has your diet been in the last 2 days", "How many hours have you slept in the last 24 hours", "Other notes" Screen 2: Data Review: Initially it will be a page that lists in some way the last n datapoints. You can click on one to see more detail (like those other less important fields). In the future, there will be a chart that shows how your mood has changed over time. I don't know if this is a separate screen or at the bottom of the data review page.
