We help students apply to colleges and universities. The form should appeal to high school students and their parents. It should be easy to read and to fill in. It would be great if students could fill it in online.

Looking for a checklist form to help students keep track of their college application to do list. It should be easy to read and to fill in. It would be great if students could fill it in online as well as on paper (8 x 10). Two pages would be okay. There should be space for students to add 12 colleges. It should have the following categories and then spaces/boxes for students to write the date they completed the task:

I would like each category to be differentiated in some way. Possibly a different color background or different color type or in a different shape. Ideally the 12 colleges would fit on one page. If that's not possible then six colleges per page would work well.
